Life care plan
A life care plan is an expert report setting out every medical service, device, medication and accommodation an injured person will need for the rest of their life, with each item costed.
Used in catastrophic cases to prove future damages, which are usually the largest component of the claim and the hardest to establish.
What is in one
Prepared by a certified life care planner, typically a nurse or rehabilitation specialist working from the medical records and treating physician opinions. It projects surgeries, therapy, medication, equipment replacement cycles, home modification and attendant care across a life expectancy, and prices each.
Why the record underneath it matters
A life care plan is only as good as the medical history it is built from. If the chronology handed to the planner is incomplete, the plan is incomplete, and a defense expert will find the gap. The planner is expensive; the records work that feeds them is not, and it determines the quality of what they produce.
