Records custodian
A records custodian is the person at a medical provider who releases records. A custodian request is the signed, formal request a firm sends to get them.
Requesting records is slow in a way that is almost entirely administrative. An authorization is signed, a request goes out, it is rejected for a formatting or date-range problem, it goes out again, and the records arrive weeks later incomplete.
Where the time actually goes
- Authorizations that expire before the request is processed
- Requests sent to the wrong entity within a hospital system
- Partial productions that omit imaging or billing
- No systematic follow-up, so a silent request stays silent
None of this is legal work, and all of it sits between signing a case and being able to value it.
